The ‘Trans Kids’ Fight Is Ending

Gradually, then suddenly. On Wednesday morning, the website of a major provider of transgender interventions for minors in California was full of detailed information about services and personnel. Late Wednesday afternoon, it looked like this:

The pediatric transgender clinic is rapidly becoming a thing of the past, even in the new forms that have recently arisen to evade a hostile political climate.

The first part of that picture is more obvious. Last week, the Department of Justice announced that it had reached settlements with two more hospitals to end “sex-rejecting procedures on children.” The agreements with New York University Langone Hospitals and University of Pittsburgh Medical Center were the fifth and sixth agreements DOJ has reached with hospitals to end transgender medical procedures for minors, an effort that included a partnership in Texas with Attorney General Ken Paxton.

Predictably, coverage of those agreements in legacy media has depicted the DOJ’s work on the topic as a bad-faith effort to intimidate hospitals and target trans-identifying individuals.

But there’s something else happening in the background, and it has so far escaped attention.

As hospitals have closed their pediatric transgender facilities, some of the doctors who practiced in those hospital-based facilities have moved to their own privately managed operations, continuing to perform the same types of mutilating surgeries and hormonal interventions. The website for the Pasadena, California, transgender facility Prosilio Care is here, and was active until Wednesday afternoon, revealing that former Los Angeles Children’s Hospital pediatrician Johanna Olson-Kennedy was on staff. Olson-Kennedy is one of the best-known and most controversial providers of transgender procedures for minors in the country, and was the medical director of the now-shuttered “Transyouth Clinic” at the Los Angeles hospital.

Prosilio was formed under the umbrella of a San Francisco facility, the Gender Confirmation Center (GCC). Prosilio advertised provision of opposite-sex hormones and mental health services, while GCC also performs transgender surgeries. Until Wednesday afternoon, staff profiles on the Prosilio website specifically mentioned services for adolescent patients:

In an effort to save pediatric transgender intervention businesses this year, Kennedy-Olson joined other providers to seek state funding for their facilities from the California legislature. That effort succeeded, and Gov. Gavin Newsom signed a state budget that included $26 million in funding to cover opposite-sex hormones and genital mutilation for minors. Remember this, because it’s about to become important: In California, transgender procedures for minors are specifically and directly funded by the state government.

Asked this week if the effort against transgender interventions on minors would also address the availability of those procedures in clinics that aren’t associated with hospitals, a DOJ spokesman told The Federalist to review an earlier press release that would “explain our position on affirmative litigation to end these practices.” That press release said that the DOJ would be guided by President Donald Trump’s January 28, 2025, order “Protecting Children From Chemical and Surgical Mutilation,” which drew no distinction over the location of those procedures:

Accordingly, it is the policy of the United States that it will not fund, sponsor, promote, assist, or support the so-called “transition” of a child from one sex to another, and it will rigorously enforce all laws that prohibit or limit these destructive and life-altering procedures.

While the Trump administration has sought to end puberty blockers and transgender surgeries for minors nationwide, additional pressures have made the provision of those services increasingly untenable. In the face of detransitioner lawsuits and federal pressure, insurance companies have become far more reluctant to provide medical malpractice insurance to the providers of transgender procedures for minors, and premiums have soared. At the same time, professional associations like the American Society of Plastic Surgeons have begun to warn doctors against participating in pediatric transgender procedures, describing the question as a matter of medical ethics.

Asked this week for comment on the legal environment for transgender procedures provided to minors, a spokesman for the Gender Confirmation Center replied that GCC “provides gender-affirming surgical care exclusively to adults,” and added this important detail: “In November 2025, GCC and Prosilio Care agreed to separate their operations effective October 1, 2026. This has been underway for nearly a year and is unrelated to recent Department of Justice actions. Over the last few months, all patients of Prosilio have … already been apprised of this change.”

In a separate message, spokesman David Perry added that Prosilio “is not transitioning to an adult-only practice. The decision was made in November 2025 to shut down Prosilio entirely, effective October 1, 2026, or sooner. That process has been underway and is unrelated to this week’s actions by the DOJ.”

While the DOJ is forcing hospitals to close facilities that engage in the mutilation of minors, the end is also coming for independent operations that aren’t affiliated with hospitals. The most aggressive and determined performers of transgender procedures for children are reading the trajectory and exiting the business.

A reporter from The Federalist visited Prosilio Care on Wednesday afternoon, and found a locked door to an apparently empty office.

The end is coming, rapidly and everywhere.
