But is this true? Here are the facts.<\/p>\n<p><strong>What does the media say?<\/strong><\/p>\n<p>The Washington Post\u2019s <a href=\"https:\/\/www.washingtonpost.com\/world\/2021\/08\/05\/global-health-rankings\/\">coverage<\/a> proved typical: \u201cU.S. health-care system ranks last among 11 high-income countries, researchers say.\u201d<\/p>\n<ul>\n<li>Numerous other news outlets \u2014 including &nbsp;<a href=\"https:\/\/thehill.com\/changing-america\/well-being\/longevity\/566715-stunning-new-report-ranks-us-dead-last-in-healthcare\">The Hill<\/a>, <a href=\"https:\/\/www.google.com\/url?sa=t&amp;rct=j&amp;q=&amp;esrc=s&amp;source=newssearch&amp;cd=&amp;cad=rja&amp;uact=8&amp;ved=2ahUKEwj4ubThs6zyAhWSZ80KHfVrB0oQxfQBMAN6BAgIEAM&amp;url=https%3A%2F%2Fwww.usnews.com%2Fnews%2Fbest-countries%2Farticles%2F2021-08-09%2Fus-ranks-last-among-11-wealthy-nations-for-health-care-study-says&amp;usg=AOvVaw0pUM4QmFRR2SptUyUuspE6\">U.S. News &amp; World Report<\/a>, and <a href=\"https:\/\/finance.yahoo.com\/news\/us-trails-behind-other-countries-in-health-care-145243663.html\">Yahoo! Finance<\/a> \u2014 carried similar headlines. The report\u2019s dour talking points were reported by websites spanned the gamut from the left-wing UK <a href=\"https:\/\/www.theguardian.com\/us-news\/2021\/aug\/05\/us-healthcare-system-ranks-last-11-wealthiest-countries\">Guardian<\/a> to the conservative outlet <a href=\"https:\/\/dailycaller.com\/2021\/08\/06\/us-healthcare-system-ranked-last-commonwealth-fund\/\">The Daily Caller<\/a>.<\/li>\n<li>Their stories are based on the latest Commonwealth Fund <a href=\"https:\/\/www.commonwealthfund.org\/sites\/default\/files\/2021-08\/Schneider_Mirror_Mirror_2021.pdf\">report<\/a>, which measures healthcare in 11 countries: Australia, Canada, France, Germany, the Netherlands, New Zealand, Norway, Sweden, Switzerland, the United Kingdom, and the United States.<\/li>\n<li>Some U.S. news outlets explicitly claimed the U.S. would do better to adopt a national healthcare system, like the UK or Canada. \u201cUnlike other countries surveyed, the United States does not have universal health coverage,\u201d The Washington Post pointed out. \u201cThe high performers stand apart from the United States in providing universal coverage and removing cost barriers \u2026 and investing in social services.\u201d<\/li>\n<\/ul>\n<p><strong>What does the report say, and is it reliable?<\/strong><\/p>\n<p>The Commonwealth Fund report \u2014 \u201cMirror, Mirror 2021: Reflecting Poorly: Health Care in the U.S. Compared to Other High-Income Countries\u201d \u2014 did, indeed, rank the U.S. last. But its rating rests on a number of questionable factors:<\/p>\n<ul>\n<li>Part of the issue is the built-in bias of the Commonwealth Fund report. \u201cThe survey is designed so that single-payer systems are strongly favoured,\u201d explained <a href=\"https:\/\/www.stmarys.ac.uk\/staff-directory\/philip-booth\">Philip Booth<\/a>, a professor of finance in the UK and a senior fellow at the free-market <a href=\"https:\/\/iea.org.uk\/\">Institute of Economic Affairs<\/a>. \u201cOne question asks whether patients have ever had disputes with their insurers. If the system is nationalised with no insurers, this simply will not happen.\u201d Booth said that previous reports have asked whether patients make co-payments at the point of service; but in national healthcare systems, patients pay via taxation before (or, too often, instead of) receiving medical treatment.<\/li>\n<li>This year\u2019s report measures \u201cequity,\u201d which \u201cfocuses on income-related disparities\u201d and \u201cdivergent\u201d outcomes. \u201cThe U.S. consistently demonstrated the largest disparities between income groups,\u201d the report states. &nbsp;But \u201cdivergent\u201d outcomes do <a href=\"https:\/\/www.youtube.com\/watch?v=pdR7WW3XR9c\">not necessarily imply bad outcomes<\/a>.<\/li>\n<li>Part of the problem comes from comparing apples to oranges. For instance, the U.S. ranks poorly on infant mortality \u2014 but that\u2019s because other nations lower their numbers by not counting some children as having being born. \u201cIs a baby born weighing less than a pound and after only 21 weeks\u2019 gestation actually \u2018born?\u2019 In some countries, the answer is no, and those births would be counted as stillbirths\u201d instead of infant mortalities, <a href=\"https:\/\/www.sciencedaily.com\/releases\/2016\/10\/161013103132.htm\">according<\/a> to Texas A&amp;M University. \u201cThese premature births are the biggest factor in explaining the United States\u2019 high infant mortality rate.\u201d<\/li>\n<li>Part of the problem is the high U.S. rate of violent crime and automobile accidents (which is linked to <a href=\"https:\/\/www.bloomberg.com\/news\/articles\/2015-04-17\/a-pew-survey-charts-global-car-motorcycle-and-bike-ownership\">high U.S. car ownership<\/a> and <a href=\"https:\/\/www.smartcitiesdive.com\/ex\/sustainablecitiescollective\/natgeo-surveys-countries%E2%80%99-transit-use-guess-who-comes-last\/9081\/\">low use<\/a> of public transportation). \u201cAs<a href=\"https:\/\/www.commentary.org\/abe-greenwald\/the-worst-study-ever\/\"> Scott Atlas<\/a>&nbsp;noted in 2011, the U.S. ranked 19th globally in life expectancy, but #1 when all variables except fatal injuries are adjusted for. Even adjusting only for differences in car fatalities and homicide, the U.S. ranked #1 in life expectancy,\u201d <a href=\"https:\/\/bongino.com\/media-pushes-deeply-flawed-study-claiming-u-s-has-worst-healthcare-system\/\">wrote<\/a> Matt Palumbo.<\/li>\n<\/ul>\n<p><strong>How does U.S. healthcare compare to other nations?<\/strong><\/p>\n<p>While the U.S. healthcare system faces real challenges, it has demonstrably better outcomes than some of the nations rated higher by the report.<\/p>\n<ul>\n<li>U.S. patients spend less time waiting to see a specialist, giving their underlying conditions less time to metastasize. In 2016, 27% of Americans had to wait a month or more to <a href=\"https:\/\/www.oecd-ilibrary.org\/sites\/242e3c8c-en\/1\/3\/2\/index.html?itemId=\/content\/publication\/242e3c8c-en&amp;_csp_=e90031be7ce6b03025f09a0c506286b0&amp;itemIGO=oecd&amp;itemContentType=book#figure-d1e391\">see a specialist<\/a>; it was 39% in Australia, 52% in Sweden, and 61% in Canada and Norway.<\/li>\n<li>The U.S. system has better outcomes on cancer than the British system. \u201cIn the UK there may be up to 15,000 avoidable deaths from cancer every year in people over the age of 75 years,\u201d <a href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(09)61214-9\/fulltext\">reported<\/a> The Lancet&nbsp; in July 2019. \u201c[B]etween 2003 and 2005, cancer mortality rates in the UK were \u2026 31% higher than in the USA among people over the age of 85 years.\u201d<\/li>\n<li>In 2017, the British Red Cross said the UK\u2019s National Health Service had created a \u201c<a href=\"https:\/\/www.theguardian.com\/society\/2017\/jan\/08\/theresa-may-denies-humanitarian-crisis-in-britains-nhs\">humanitarian crisis<\/a>\u201d thanks to long waits, overcrowded facilities, and understaffed offices.<\/li>\n<li>Americans spend much less time waiting in the emergency room than other nations.<\/li>\n<li>In the pre-pandemic U.S., 74% of patients spent less than an hour waiting in the emergency room for treatment, and 44% of Americans waited less than 15 minutes, <a href=\"https:\/\/www.cdc.gov\/nchs\/data\/nhamcs\/web_tables\/2018-ed-web-tables-508.pdf\">according<\/a> to the CDC. At the same time, Canadians waited <a href=\"https:\/\/ipolitics.ca\/2019\/11\/28\/emergency-department-wait-times-on-the-rise-in-canada-cihi-data\/\">3.2 hours<\/a>, and the UK has not met its goal of having 95% of patients admitted within <a href=\"https:\/\/www.bbc.com\/news\/health-50397856\">four hours<\/a> in years. More than 5,000 British patients had to wait more than an hour <a href=\"https:\/\/news.sky.com\/story\/nhs-winter-crisis-thousands-of-ambulance-delays-11203206\">inside the ambulance<\/a> before being taken inside the emergency room.<\/li>\n<li>\u201cIn all 5,449 people have lost their lives since 2016 as a direct result of waiting anywhere between six hours and 11 hours\u201d in UK accident and emergency (A&amp;E) departments, <a href=\"https:\/\/www.theguardian.com\/society\/2019\/dec\/10\/thousands-of-patients-die-waiting-for-beds-in-hospitals-study\">reported<\/a> the Guardian.<\/li>\n<li>The average wait for a knee replacement is <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/8090168\/\">twice as long<\/a> in Canada as in the U.S.<\/li>\n<\/ul>\n<p><strong>What about healthcare rationing?<\/strong><\/p>\n<p>Critics of the U.S. healthcare system often accuse it of \u201crationing,\u201d as insurance companies decline to pay for certain treatments. For instance, the Commonwealth Fund report scored the U.S. worst for the number of people who said that their \u201c[i]nsurance denied payment for medical care or did not pay as much as expected.\u201d<\/p>\n<p>While high deductibles (which have <a href=\"https:\/\/www.aei.org\/articles\/the-aca-trillions-yes-a-revolution-no\/\">more than doubled<\/a> since the passage of the Affordable Care Act) and reduced benefits cause headaches of their own, they do not compare to genuine rationing. National healthcare systems can deny treatment outright:<\/p>\n<ul>\n<li>The NHS denied care to infants Charlie Gard and Alfie Evans, and it fought a legal battle to prevent their parents from taking them to other healthcare providers who offered their services free of charge.<\/li>\n<li>The NHS denied treatment to cancer victims like <a href=\"https:\/\/www.express.co.uk\/news\/uk\/623203\/Two-year-old-suffering-from-cancer-forced-to-fly-to-US-for-treatment-NHS-refused-to-give\">Alexander Vinson<\/a>, age 2, and <a href=\"https:\/\/www.bbc.com\/news\/uk-england-berkshire-49496837\">Charlie Ilsey<\/a>, age 12, who sought treatment elsewhere.<\/li>\n<li>If the government does not forbid them, families with the means may travel abroad for treatment (often <a href=\"https:\/\/www.dailywire.com\/news\/saved-socialism-us-saves-baby-oliver-after-uk-amanda-prestigiacomo\">to the U.S.<\/a>), while poor families wait at home to die. (Talk about \u201cdisparities\u201d and \u201cinequities.\u201d)<\/li>\n<li>An investigation by London\u2019s Sunday Times <a href=\"https:\/\/www.thetimes.co.uk\/article\/revealed-how-elderly-paid-price-of-protecting-nhs-from-covid-19-7n62kkbtb?region=global\">found<\/a> that the NHS kept COVID-19 hospital admissions artificially low by withholding treatment from the elderly based on an \u201cage-based frailty score.\u201d The NHS contests this allegation.<\/li>\n<\/ul>\n<p>This denial of treatment inherent in any socialized medicine system may explain why the U.S. rated highest in the report for the number of chronically ill patients who \u201cdiscussed with [a] health professional their main goals and priorities in caring for their condition,\u201d as well as their \u201ctreatment options,\u201d over the past year.<\/p>\n<p><strong>What\u2019s the verdict?<\/strong><\/p>\n<p>The U.S. does not have the worst healthcare system of the developed world. The Commonwealth Fund report \u201clacks context\u201d by skewing its results toward nations that embrace national healthcare systems.<\/p>\n<p>Of course, this is not to say that America\u2019s increasingly government-dominated healthcare sector is without its share of challenges. The U.S. has a very real problem with <a href=\"https:\/\/www.marketwatch.com\/story\/the-us-is-the-most-obese-nation-in-the-world-just-ahead-of-mexico-2017-05-19\">obesity<\/a>, <a href=\"https:\/\/www.sciencedaily.com\/releases\/2019\/02\/190221083419.htm\">addiction<\/a>, and <a href=\"https:\/\/www.usnews.com\/news\/best-countries\/articles\/2019-02-07\/global-suicide-rate-declines-while-us-rate-rises-study-finds\">suicide<\/a>.
