The 86-page document <\/span><a href=\"https:\/\/www.ama-assn.org\/system\/files\/2021-05\/ama-equity-strategic-plan.pdf\"><span>outlines<\/span><\/a><span> a three-year plan to implement \u201canti-racist\u201d initiatives, including pushing \u201ccritical race theory\u201d specifically in the medical community.<\/span><\/p>\n<p><span>The plan announces a five-pronged approach to embracing equity. The AMA makes a commitment to \u201cembed racial and social justice throughout\u201d the organization\u2019s policies and practices, and promises to \u201cbuild alliances and share power with historically marginalized minoritized physicians.\u201d<\/span><\/p>\n<p><span>The organization also promises to \u201censure equitable structures and opportunities in innovation\u201d for marginalized communities and minorities, to \u201cpush upstream to address all determinants of health and the root causes of health inequities,\u201d and to \u201cfoster truth, racial healing, reconciliation and transformation\u201d regarding the AMA\u2019s own past.<\/span><\/p>\n<p><span>One of the plan\u2019s specific goals is to expand medical school and physician education to include \u201cequity, anti-racism, structural competency, public health and social sciences, critical race theory and historical basis of disease.\u201d The plan pushes mandatory \u201canti-racism\u201d and \u201cequity-explicit\u201d training for all medical trainees and staff.<\/span><\/p>\n<p><span>The plan also includes elements that appear to have little to do with medicine or the medical community, including a declaration dubbed the \u201cLand and Labor Acknowledgement.\u201d<\/span><\/p>\n<p><span>\u201cWe acknowledge that we are all living off the taken ancestral lands of Indigenous peoples for thousands of years,\u201d the AMA\u2019s declaration reads.<\/span><\/p>\n<p><span>\u201cWe acknowledge the extraction of brilliance, energy and life for labor forced upon people of African descent for more than 400 years. We celebrate the resilience and strength that all Indigenous people and descendants of Africa have shown in this country and worldwide. We carry our ancestors in us, and we are continually called to be better as we lead this work.\u201d<\/span><\/p>\n<p><span>The equity plan slams the \u201cmyth of meritocracy,\u201d saying the differences in outcomes among historically marginalized populations and minorities have been \u201cexplained away\u201d as the results of a meritocracy.<\/span><\/p>\n<p><span>\u201cIt is a narrative that attributes success or failure to individual abilities and merits. It does not address the centuries of unequal treatment that have intentionally robbed entire communities of the vital resources needed to thrive,\u201d the AMA says.<\/span><\/p>\n<p><span>Medical schools have adopted dramatic affirmative action policies in order to matriculate a \u201ccritical mass\u201d of black applicants, who tend to score lower on the Medical College Admission Test (MCAT) than their white and Asian counterparts.<\/span><\/p>\n<p><span>Lowering standards for those providing life-saving care could have dangerous consequences.<\/span><\/p>\n<p><span>In 2015-2016, an MCAT score of 24-26 and a GPA of 3.20 to 3.39 was only good enough to gain admission to medical school for 8% of white people, but more than 56% of black people with those scores got in, according to an <\/span><a href=\"https:\/\/www.aei.org\/carpe-diem\/acceptance-rates-at-us-medical-schools-in-2015-reveal-ongoing-discrimination-against-asian-americans-and-whites\/\"><span>American Enterprise Institute<\/span><\/a><span> review of AMA data.<\/span><\/p>\n<p><span>Seventy percent of the time, administrators said a white person with an MCAT score of 27-29 and a GPA of 3.40-3.59 would not make a suitable doctor. But more than 80% of black people with those scores were admitted. The AMA lists events that it says perpetrated \u201cstructural racism\u201d against minorities.<\/span><\/p>\n<p><span>One is what it calls \u201cJohnson v. DeGandy\u201d of 1992, apparently referring to <\/span><i><span>Johnson v. DeGrandy<\/span><\/i><span> of 1994, a court case about how congressional districts are drawn.<\/span><\/p>\n<p><span>Another is the 1996 Supreme Court decision it calls <\/span><i><span>Roman v. Evans.<\/span><\/i><span> The Colorado governor\u2019s name in that case was Roy Romer, not Roman. The court <\/span><a href=\"https:\/\/supreme.justia.com\/cases\/federal\/us\/517\/620\/\"><span>decided<\/span><\/a><span> in the case that Colorado was violating the Constitution by preventing protected status based upon homosexuality.<\/span><\/p>\n<p><span>The document positions the Supreme Court <\/span><a href=\"https:\/\/supreme.justia.com\/cases\/federal\/us\/532\/275\/\"><span>decision<\/span><\/a><span> in <\/span><i><span>Alexander v. Sandoval <\/span><\/i><span>in a graphic purporting to illustrate the progress and setbacks against racism against Hispanics. But it positions it at 1992, when it occurred in 2001. That decision said that the Civil Rights Act only allows private lawsuits over intentional discrimination, not disparate impact on minorities.<\/span><\/p>\n<p><span>The AMA claims that racism and discrimination ensure that \u201chistorically marginalized communities remain disproportionately plagued by early disease and abbreviated life.\u201d According to the Centers for Disease Control and Prevention (CDC), however, Hispanics in the U.S. have a life expectancy about <\/span><a href=\"https:\/\/abcnews.go.com\/WN\/us-hispanics-longer-life-expectancy-white-black-americans\/story?id=11883156\"><span>three years<\/span><\/a><span> longer than whites.<\/span><\/p>\n<p><span>In February, the AMA found itself embroiled in controversy over a since-removed podcast that was posted on the Journal of the AMA\u2019s (JAMA) website, titled \u201cStructural Racism for Doctors\u2014What Is It?\u201d The podcast featured two Jewish doctors, Ed Livingston, JAMA\u2019s deputy editor for clinical content, and Mitchell Katz, an editor at JAMA Internal Medicine and CEO of NYC Health and Hospitals.<\/span><\/p>\n<p><span>Livingston <\/span><a href=\"https:\/\/www.dailywire.com\/news\/medical-journal-forces-out-doctor-for-questioning-structural-racism\"><span>asked<\/span><\/a><span> on the podcast, \u201cGiven that racism is illegal, how can it be so embedded in society that it\u2019s considered structural?\u201d<\/span><\/p>\n<p><span>\u201cPeople do have this response which we\u2019ve said repeatedly, \u2018I\u2019m not a racist, so why are you calling me a racist.\u2019 And because they respond that way, they are turned off by the whole structural racism phenomenon. Are there better terms we can use?\u201d he said.<\/span><\/p>\n<p><span>Livingston was asked to resign, and Katz apologized, but was put on administrative leave.<\/span><\/p>\n<p><span>The AMA apologized in its equity plan for its \u201cegregious, harmful error and failure in posting a podcast denying structural racism; keeping old wounds open while new ones form.\u201d<\/span><\/p>\n<p><span>The American Heart Association (AHA) <\/span><a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/CIR.0000000000000936\"><span>adopted<\/span><\/a><span> a similar anti-racist ideology, <\/span><a href=\"https:\/\/www.dailywire.com\/news\/medical-professor-warned-colleagues-that-schools-racial-preferences-were-illegal-and-he-was-removed-from-directorship-now-hes-suing\"><span>prompting<\/span><\/a><span> a lawsuit last year over disciplinary action taken against an Asian American doctor who questioned whether using race in admissions decisions is legal.<\/span><\/p>\n<p><span>University of Pittsburgh School of Medicine professor Norman Wang published a white paper in the Journal of the American Heart Association (JAHA) arguing that affirmative action has not actually helped black and Hispanic students.
