(Franklin County Sheriff&rsquo;s Office)<\/figcaption><\/figure>\n<\/p><\/div>\n<p>In 2022, an <a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/web.archive.org\/web\/20250124105221\/https:\/\/www.washingtonpost.com\/archive\/lifestyle\/1997\/02\/13\/the-saga-of-pretty-boy-pedro\/32910dec-daf8-4f75-b6f3-14a27910885b\/\">infamous<\/a> white supremacist who identifies as a woman was the <a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/sparrowmedia.net\/2023\/02\/donna-langan\/\">first<\/a> federal prisoner to undergo reconstructive surgery, namely<a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/storage.courtlistener.com\/recap\/gov.uscourts.dcd.235987\/gov.uscourts.dcd.235987.1.0.pdf\"> vaginoplasty<\/a>, the construction of a pseudo-vagina out of inverted penile tissue.<\/p>\n<p><a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/www.advocate.com\/transgender\/2015\/11\/06\/women-san-quentin#rebelltitem4\">Peter Kevin Langan<\/a>, now known as &ldquo;Donna,&rdquo; is the former <a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/www.documentcloud.org\/documents\/21635596-langan-declaration\/\">ringleader<\/a> of the Aryan Republican Army, a terrorist neo-Nazi gang <a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/www.splcenter.org\/resources\/stories\/terror-right\/\">suspected<\/a> of financing the <a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/www.fbi.gov\/history\/famous-cases\/oklahoma-city-bombing\">1995 Oklahoma City bombing<\/a> by funneling funds stolen from <a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/www.latimes.com\/archives\/la-xpm-1996-03-30-mn-52897-story.html\">a string of armed bank robberies<\/a>.  <\/p>\n<p>Langan presently is <a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/www.govinfo.gov\/content\/pkg\/USCOURTS-ohsd-2_02-cv-00832\/pdf\/USCOURTS-ohsd-2_02-cv-00832-0.pdf\">serving<\/a> a life sentence, post-operation, at a special needs women&rsquo;s prison in Texas.<\/p>\n<p>The newly issued BOP policy is expected to generate at least $1.6 million in potential savings from surgeries alone, based on estimates from eight existing surgical requests.<\/p>\n<p>As for hormone therapy, related expense reductions will be quantifiable as savings are realized, a DOJ official told the <em>Washington Examiner<\/em>.<\/p>\n<p>&ldquo;The new policy will result in almost $2 million not being spent on otherwise harmful treatment that also results in safety concerns at BOP facilities,&rdquo; the DOJ official said.<\/p>\n<p><a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/www.washingtonexaminer.com\/news\/investigations\/3890471\/trans-inmates-separated-from-females-following-sexual-abuse-claims\/\"><strong>TRANSGENDER INMATES SEPARATED FROM FEMALES AT SPECIAL NEEDS WOMEN&rsquo;S PRISON FOLLOWING SEXUAL ABUSE CLAIMS<\/strong><\/a><\/p>\n<p>In 2022, former President Joe Biden&rsquo;s BOP reinstated the Transgender Offender Manual, which was originally released by the Obama-Biden administration in January 2017, but <a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/embed.documentcloud.org\/documents\/4327113-Bureau-of-Prisons-Transgender-Offender-Manual\/\">that version<\/a> had only made hormone medication accessible at the time.<\/p>\n<p>President Donald Trump&rsquo;s BOP <a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/platform.vox.com\/wp-content\/uploads\/sites\/2\/chorus\/uploads\/chorus_asset\/file\/10835485\/transgender_prison_guidelines.0.pdf\">repealed<\/a> much of the manual&rsquo;s provisions in 2018, but BOP officials reissued the handbook when Biden took office and <a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/www.documentcloud.org\/documents\/25502540-bop-transgender-offender-manual-2022-version\/\">updated<\/a> it to include surgical consideration.<\/p>\n<p>Per the recently revised Trump BOP guidelines, titled &ldquo;Management of Inmates with Gender Dysphoria,&rdquo; prison health services will instead prescribe anti-depressants and psychotherapy to address underlying feelings of bodily discomfort and any co-morbid mental health disorders. A diagnosis of gender dysphoria, a perceived incongruence of one&rsquo;s sense of sex, is <a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0165178122004875\">often accompanied<\/a> by other psychiatric conditions.<\/p>\n<p>The recommended treatment intends to alleviate psychological distress in those <a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/www.psychiatry.org\/patients-families\/gender-dysphoria\/what-is-gender-dysphoria\">clinically diagnosed with gender dysphoria<\/a> through therapeutic remedies, rather than resorting to physical intervention.<\/p>\n<p>For inmates already on a hormone-replacement regimen, BOP will consider placing them on a taper plan that gradually reduces their dosage at a safe withdrawal pace. Tapering will be reevaluated regularly and adjusted accordingly based on monitoring.<\/p>\n<p>The policy also cuts off <a href=\"https:\/\/www.conservativenewsdaily.net\/breaking-news\/op-ed-who-really-runs-washington\/\" title=\"Op-Ed: Who Really Runs Washington?\">taxpayer-funded access<\/a> to nonmedical accommodations, such as breast padding, chest binders, crouch packers, female undergarments, makeup, and wigs, provided for free as part of an inmate&rsquo;s &ldquo;social transitioning&rdquo; process.<\/p>\n<p>Prison staff shall confiscate all free-of-charge social accommodations currently in use. Similar items, such as cosmetics and general gender-specific clothing, however, are still available for purchase on the <a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/www.bop.gov\/locations\/institutions\/phx\/phx_commlist.pdf?v=1.0.1\">commissary shopping list<\/a>.<\/p>\n<p>The policy is not fully in effect either. Pending the resolution of <em>Kingdom v. Trump<\/em>, <a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/www.aclu.org\/press-releases\/federal-judge-temporarily-enjoins-federal-prison-officials-from-withholding-health-care-from-incarcerated-trans-people\">a federal lawsuit<\/a> brought by the American Civil Liberties Union against BOP, officials must comply with a <a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/assets.aclu.org\/live\/uploads\/2025\/06\/Kingdom-decision-PI-and-class-cert.pdf\">preliminary injunction<\/a> temporarily blocking the enforcement of Trump&rsquo;s <a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/www.whitehouse.gov\/presidential-actions\/2025\/01\/defending-women-from-gender-ideology-extremism-and-restoring-biological-truth-to-the-federal-government\/\">executive order<\/a> on &ldquo;Restoring Biological Truth to the Federal Government.&rdquo;<\/p>\n<p>Trump had instructed, by presidential decree, the bureau to ensure that &ldquo;no federal funds are expended&rdquo; for the purpose of conforming a prisoner&rsquo;s outward appearance to that of their desired sex.<\/p>\n<p>Notably, the injunctive relief, though extended classwide in the <em>Kingdom<\/em> case, does not require BOP to provide &ldquo;gender-affirming surgeries.&rdquo;<\/p>\n<p><strong><a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/www.washingtonexaminer.com\/news\/investigations\/4452277\/trump-administration-treading-carefully-transgender-prison-housing\/\">TRUMP ADMINISTRATION TREADING CAREFULLY IN TRANSGENDER PRISON HOUSING CASES<\/a><\/strong><\/p>\n<p>LGBT activists have already equated the policy to an act of genocide.<\/p>\n<p>&ldquo;People will die,&rdquo; an advocate <a target=\"_blank\" rel=\"noopener noreferrer\" href=\"https:\/\/www.themarshallproject.org\/2026\/02\/19\/transgender-federal-prisons-care-ban-policy\">told<\/a> The Marshall Project. &ldquo;People will die by suicide.
