The woman experienced episodes of pain over \u2062a significant period of time\u2063 following her surgery\u2063 until the AWR \u2062was removed in 2021,\u201d McDowell said in the report. \u201cI accept her concerns regarding \u200dthe impact this had on her health and wellbeing and \u200cthat of her family.\u201d<\/p>\n<\/blockquote>\n<p>According to the report, a large wound retractor was utilized during the \u2063C-section procedure, but the\u200b surgeon thought it was \u200ctoo small and requested \u2062an\u2063 extra-large model.<\/p>\n<p>When another CT scan was performed more than a year later, it was this second, much larger device that had been left within the patient.<\/p>\n<p>The complaint further stated that the retractors were not accounted \u200bfor when medical equipment was \u2063tallied in the hospital department.<\/p>\n<p>The Auckland health authority, also known as Te Whatu \u2063Ora Te Toka Tumai Auckland, doubled down by stating that\u2062 it had\u200b correctly performed its duties by\u2064 informing \u200cpersonnel that the retractors\u200b should be included in the count.<\/p>\n<blockquote class=\"wp-block-quote\">\n<p>\u201cI would like to \u2063say how sorry we are\u200b for what happened to\u2064 the patient, and acknowledge \u200dthe impact that this will have had on\u2063 her and her wh\u0101nau [family]. For ethical and privacy reasons we can\u2019t comment\u200b on the details of individual patient \u2064care,\u201d said\u2062 Mike Shepard, Te Whatu \u2064Ora\u2019s group director of operations for Aukland. \u201cHowever, we have reviewed the patient\u2019s\u200c care and this has resulted in improvements to our \u200dsystems and processes which will reduce the chance of \u2063similar incidents happening again.\u201d<\/p>\n<\/blockquote>\n<p>The director of procedures for the commissioner has\u200c now been handed the case, and they have the authority to\u2063 file disciplinary actions that might eventually\u200c lead to the termination of individual employees who were involved in the \u2064medical error.<\/p>\n<p>However, this is not a unique incidence.<\/p>\n<p>In 2021,\u2063 a <a href=\"https:\/\/www.conservativenewsdaily.net\/breaking-news\/teen-girl-charged-with-hate-crime-enhancement-after-allegedly-crumpling-back-the-blue-sign-in-front-of-deputy\/\" title=\"Teen Girl Charged With Hate Crime Enhancement After Allegedly Crumpling \u2018Back The Blue\u2019 Sign In Front Of Deputy\">similar incident occurred<\/a> in the Auckland \u2064suburb of Waitemat when an\u200b Alexis wound retractor was introduced\u200b into\u2064 a patient\u2019s stomach \u200cduring\u200d emergency surgery to repair a perforated colon.
