Still, advocates have rejected the process being described as \u201cchemical castration,\u201d instead opting for the euphemism \u201cgender-affirming care.\u201d<\/p>\n<p>\u201cSo before getting castrated physically, you can actually do a chemical castration, kind of, as a trial,\u201d said Dr. Michael Irwig, an endocrinologist who contributed to the chapter. He said he\u2019s treated two people who identify as eunuchs.<\/p>\n<p>The comments came amid the debut of new <a href=\"https:\/\/www.tandfonline.com\/doi\/full\/10.1080\/26895269.2022.2100644\" target=\"_blank\" rel=\"noopener\">Standards of Care guidelines<\/a>, which included the unprecedented addition of a chapter that seeks to include \u201ceunuch\u201d under the \u201ctransgender umbrella.\u201d The inclusion of eunuchs may make it easier for candidates to access castration to \u201caffirm\u201d their \u201cgender identity.\u201d<\/p>\n<p>Irwig said a chemical cocktail of hormones and blockers can be used to chemically castrate eunuchs. He did not explicitly say whether dosages for the gender-affirming care of \u201ceunuchs\u201d would be the same as for the treatment of gender dysphoric minors.<\/p>\n<p>\u201cSo there are ways to suppress testosterone levels, through hormonal suppression, to explore the effects of androgen deficiency for eunuch individuals wishing to become, for example, asexual or nonsexual or androgynous,\u201d said Irwig.<\/p>\n<p>Endocrinologist Dr. Mike Laidlaw told <a href=\"https:\/\/www.dailywire.com\" target=\"_blank\" rel=\"noopener\">The Daily Wire<\/a> that \u201cchemical castration\u201d is, effectively, when a medication like Lupron is used to stop the pituitary from communicating with the testicles or ovaries to drastically lower sex hormone levels. It is no different from the \u201cgender affirming care\u201d given to adolescents, Laidlaw said.<\/p>\n<p>\u201cIt\u2019s just that the effects are more dramatic, and I would say more dangerous, in puberty,\u201d said Laidlaw.<\/p>\n<p>WPATH did not respond to The Daily Wire\u2019s request for comment.<\/p>\n<p>In April, Politifact issued a <a href=\"https:\/\/www.politifact.com\/factchecks\/2022\/apr\/17\/mary-miller\/rep-mary-miller-says-white-house-encouraging-kids-\/\" target=\"_blank\" rel=\"noopener\">fact-check<\/a> of Rep. Mary Miller (R-IL) after she said the Biden administration was encouraging the chemical castration of minors. The organization rated her statement as \u201cfalse.\u201d<\/p>\n<p>\u201cExperts we spoke with said [chemical castration] is not a medical term and not commonly used by medical professionals, but generally seems to be used to describe hormone therapy,\u201d Politifact wrote.<\/p>\n<p>Politifact then claimed puberty blockers are approved by the FDA, which is not entirely true. While gonadotropin-releasing hormone (GnRH) agonists are approved for other uses, the FDA has not approved puberty blockers for their off-label use to treat gender dysphoria. Off label, doctors have prescribed puberty blockers as a \u201ctreatment\u201d for gender dysphoria, given during adolescence \u2013 a period of rapid growth and development \u2013 on an experimental basis. There are no long-term studies to support its use in this context.<\/p>\n<p>The FDA recently issued a <a href=\"https:\/\/publications.aap.org\/aapnews\/news\/20636\/Risk-of-pseudotumor-cerebri-added-to-labeling-for?autologincheck=redirected\" target=\"_blank\" rel=\"noopener\">warning<\/a> for GnRH agonists after having identified a serious potential side effect for some that may cause a dangerous surge of spinal fluid pressure in the brain. This build-up can cause headaches, nausea, double vision, and even permanent vision loss.<\/p>\n<p>GnRH agonists, sold under the brand name Lupron, among others, are only <a href=\"https:\/\/www.accessdata.fda.gov\/drugsatfda_docs\/label\/2018\/020708s035lbl.pdf\" target=\"_blank\" rel=\"noopener\">FDA approved<\/a> for endometriosis, fibroids, precocious puberty, and as a chemotherapy drug for prostate cancer. It is also sometimes used to perform chemical castration of violent sex offenders.<\/p>\n<p>The inclusion of the eunuch gender identity was a bizarre choice, Laidlaw says, and negates the cogency of \u201cgender identity\u201d as a concept. \u201cWith this addition, WPATH has inadvertently killed off the \u2018gender identity\u2019 as a serious concept in psychology.\u201d<\/p>\n<p>\u201cNo one will ever find a corner of the brain where the \u2018eunuch identity\u2019 is hidden. They will also never find any sort of \u2018gender identity\u2019 in the brain,\u201d said Laidlaw. \u201cIt is a phantom with no basis in physical reality.
